Bereavement services forms

From 1 March 2025, statutory forms must be used to apply for a burial at any burial ground in Scotland.

The new statutory forms and related guidance to be used as a requirement of The Burial (Applications and Register) (Scotland) Regulations 2024 are available at Scottish Government - Burial: Statutory forms.

The forms can be completed electronically or printed and completed by hand.
