Short-Term Assessment and Rehabilitation Facility

This is a residential facility which will support older people on a 24-hour basis for a short period of time, depending on need.

The purpose of the facility is to enable older people to be discharged from hospital quickly to a homely environment and to prevent people being admitted to hospital inappropriately.

In this facility residents will be assessed and receive support to return home with a package of care or, if they require 24-hour care on a permanent basis, they will be supported in this transition and in finding a care home of their choice.

This facility has both Health and Social Care staff attached, including District Nurses, Occupational Therapist, Community Mental Health Team and the local General Practitioners.
