Hospital Social Work Team

WHO ARE WE ?

We are a team of Social Services Staff based in Crosshouse Hospital.

We can offer assistance to you or your family in relation to difficulties caused by being in hospital or with other concerns you may have.
Our work is with patients from East Ayrshire, North Ayrshire and South Ayrshire.
We can help you with:

• a range of emotional and practical supports
• difficulties in adjusting to a change in your health
• need for help at home
• worries about loss of independence
• difficulties in caring for yourself or others
• relationship difficulties
• housing issues
• feelings of isolation
• Benefits / Financial issues
• Future Care Needs

WHAT KIND OF HELP DO WE PROVIDE ?

We offer a range of assistance including:

• Provision of Information Advice
• Support Care at Home Counselling
• Advocacy Benefits Advice
• Income Maximisation
• Assessment of Need
• Access to Equipment and Adaptations

Alternatively we may be able to put you in touch with other Professionals or agencies who can assist. Our work is aimed at helping plan for your discharge or in longer term illness, to offer ongoing assistance.
We are able to meet with you either in the hospital or where appropriate, arrange to visit you or your family at home.

9.00am - 5.00pm Monday to Thursay
9.00am - 4.00pm on a Friday

Please note that we operate an appointment system but are available in Emergency Situations during office hours.

Where possible we would hope to deal with your difficulty ourselves. In some cases, with your permission, we would ask other agencies or social work colleagues to assist.
